Cet organe f\u00e9minin a pour fonction de recevoir le p\u00e9nis durant le co\u00eft et de servir aussi bien de conduit d\u2019\u00e9vacuation \u00e0 la menstruation que de canal (inf\u00e9rieur) lors de l\u2019accouchement.<\/p> <p>L\u2019ut\u00e9rus, \u00e9galement appel\u00e9 matrice ou sein maternel, est l\u2019organe dans lequel a lieu la gestation. C\u2019est le plus grand organe de l\u2019appareil reproducteur f\u00e9minin. Il s\u2019agit d\u2019un organe musculaire, creux, en forme de poire, situ\u00e9 entre le col de l\u2019ut\u00e9rus et les trompes de Fallope. Son int\u00e9rieur est recouvert d\u2019une membrane appel\u00e9e endom\u00e8tre.<\/p> <p><!--more--><\/p> <p>Les trompes de Fallope sont des conduits musculaires qui relient l\u2019ut\u00e9rus ou la matrice aux ovaires.<\/p> <p>Les ovaires sont des organes de la taille d\u2019une noix et constituent la partie de l\u2019organe reproducteur f\u00e9minin charg\u00e9e de produire et de secr\u00e9ter les hormones sexuelles et les ovules. Il s\u2019agit de structures en forme d\u2019amande qui ont chez la femme fertile les dimensions suivantes : 1 x 2 x 3 cm (leur taille variant au cours du cycle). Ils p\u00e8sent environ 6 \u00e0 7 grammes et sont de couleur blanc-gris\u00e9. Ils sont fix\u00e9s de chaque c\u00f4t\u00e9 de l\u2019ut\u00e9rus par les ligaments ut\u00e9ro-ovariens et \u00e0 la paroi pelvienne par les infundibulum pelviens. Les ovaires de la femme sont l\u2019\u00e9quivalent des testicules chez l\u2019homme.<\/p> <h2 class=\"font-bold font-sans text-black text-2xl lg:text-3xl leading-tight\">\u00a0<\/h2> <h2 class=\"font-bold font-sans text-black text-2xl lg:text-3xl leading-tight\">Les ovules et l&rsquo;ovulation<\/h2> <p>Les ovules sont les cellules sexuelles f\u00e9minines (ou gam\u00e8tes f\u00e9minins). Il s\u2019agit de cellules microscopiques (environ 80 microns) qui sont cependant beaucoup plus grandes que les spermatozo\u00efdes. Elles sont sph\u00e9riques et immobiles.<\/p> <p>Tous les mois (environ tous les 28 jours), \u00e0 partir de la pubert\u00e9 et jusqu\u2019\u00e0 la m\u00e9nopause, plusieurs ovules vont entamer une \u00e9tape de maturation dans les ovaires au sein de petits \u00ab kystes \u00bb remplis de liquides appel\u00e9s follicules primordiaux, ce processus \u00e9tant appel\u00e9e l\u2019ovogen\u00e8se. Un seul de ces follicules (qui contiennent une ovule) deviendra dominant, tandis que les autres s\u2019atrophieront et seront absorb\u00e9s par l\u2019ovaire elle-m\u00eame, et ne pourront plus servir ensuite.<\/p> <p>Vers environ la moiti\u00e9 du cycle menstruel de la femme, le follicule dominant lib\u00e8rera un seul ovule au moment de l\u2019ovulation qui transitera par la trompe de Fallope en direction de l\u2019ut\u00e9rus.<\/p> <h2 class=\"font-bold font-sans text-black text-2xl lg:text-3xl leading-tight\">\u00a0<\/h2> <h2 class=\"font-bold font-sans text-black text-2xl lg:text-3xl leading-tight\">La r\u00e9serve ovarienne<\/h2> <p>La quantit\u00e9 d\u2019ovules ou, ce qui revient au m\u00eame, de follicules primordiaux, dont dispose la femme, est appel\u00e9e la r\u00e9serve ovarienne et celle-ci est fix\u00e9e d\u00e8s avant sa naissance puisque, en effet, c\u2019est vers la 20e semaine de gestation que la femme, avant sa naissance, aura le plus grand nombre de follicules primordiaux (pr\u00e8s de 7 millions).<\/p> <p>\u00c0 partir de ce moment-l\u00e0, le nombre de follicules diminuera peu \u00e0 peu et ces derniers ne seront pas remplac\u00e9s. La femme na\u00eetra avec environ 2 millions de follicules primordiaux et il lui en restera environ 400 000 lors de sa premi\u00e8re menstruation.<\/p> <p>De nos jours, l\u2019\u00e9valuation de la r\u00e9serve ovarienne d\u2019une femme constitue un \u00e9l\u00e9ment fondamental pour un sp\u00e9cialiste de la reproduction \u00e0 l\u2019heure d\u2019\u00e9tablir le pronostic reproductif d\u2019une patiente. Cette \u00e9valuation repose sur des donn\u00e9es extr\u00eamement utiles comme peut l\u2019\u00eatre le d\u00e9nombrement de follicules antraux rendu possible gr\u00e2ce \u00e0 une \u00e9chographie haute r\u00e9solution ou l\u2019\u00e9valuation hormonale de l\u2019hormone FSH ou de l\u2019hormone antim\u00fcll\u00e9rienne (AMH).<\/p> <h2 class=\"font-bold font-sans text-black text-2xl lg:text-3xl leading-tight\">\u00a0<\/h2> <h2 class=\"font-bold font-sans text-black text-2xl lg:text-3xl leading-tight\">Le cycle menstruel et l\u2019ovulation<\/h2> <p>La glande pituitaire du cerveau est charg\u00e9e de produire et de doser les hormones de contr\u00f4le des diff\u00e9rentes \u00e9tapes qui conduisent \u00e0 l\u2019ovulation. Les deux hormones qui r\u00e9gulent ce processus sont les suivantes :<\/p> <ul> <li>L\u2019hormone folliculostimulante (ci-apr\u00e8s FSH)<\/li> <li>L\u2019hormone lut\u00e9inisante (ci-apr\u00e8s LH)<\/li> <\/ul> <p>\u00a0<\/p> <p>Le mode de lib\u00e9ration de ces hormones permet de diviser le cycle menstruel en trois phases :<\/p> <ul> <li><strong>Phase folliculaire :<\/strong> Elle commence le 1er jour du cycle menstruel ou, ce qui revient au m\u00eame, le premier jour du saignement menstruel. L\u2019hormone FSH commence par stimuler le d\u00e9veloppement de plusieurs follicules mais, \u00e9tant donn\u00e9 que les taux de FSH diminuent progressivement lors d\u2019un cycle naturel, seul un follicule sera dominant et produira un ovule mature. Les follicules non dominants seront absorb\u00e9s par l\u2019ovaire et ne pourront pas \u00eatre r\u00e9utilis\u00e9s lors des cycles post\u00e9rieurs (autrement dit, ils sont perdus). D\u2019autre part, le follicule en d\u00e9veloppement va s\u00e9cr\u00e9ter ensuite des \u0153strog\u00e8nes qui auront, entre autres, la fonction de contribuer aux modifications n\u00e9cessaires du mucus cervical afin d\u2019aider les spermatozo\u00efdes \u00e0 passer dans l\u2019ut\u00e9rus ou encore de provoquer l\u2019\u00e9paississement n\u00e9cessaire de l\u2019endom\u00e8tre pour que l\u2019embryon puisse s\u2019y implanter. L\u2019\u0153strog\u00e8ne principal produit par l\u2019ovaire est l\u2019\u0153stradiol (E2).<\/li> <li><strong>Phase ovulatoire :<\/strong> Il s\u2019agit d\u2019une phase de courte dur\u00e9e. Elle d\u00e9bute lorsque, en r\u00e9ponse \u00e0 l\u2019augmentation des taux d\u2019\u0153strog\u00e8nes, le taux de LH augmente de fa\u00e7on spectaculaire. Ce pic de LH entra\u00eene la maturation finale de l\u2019ovule, la rupture du follicule et la lib\u00e9ration de l\u2019ovule. Cette phase se d\u00e9roule normalement 14 jours avant la menstruation suivante ou, ce qui revient au m\u00eame, le 14e jour d\u2019un cycle de 28 jours.<\/li> <li><strong>Phase lut\u00e9ale :<\/strong> Elle d\u00e9bute apr\u00e8s l\u2019ovulation. Arriv\u00e9 \u00e0 ce point, le follicule ovarien qui a lib\u00e9r\u00e9 l\u2019ovule au cours de la phase pr\u00e9c\u00e9dente se paralyse et se solidifie pour se transformer en ce que l\u2019on appelle le corps lut\u00e9al. Cette structure est tr\u00e8s importante car elle produit la progest\u00e9rone, autrement dit, l\u2019hormone charg\u00e9e de transformer et de pr\u00e9parer l\u2019endom\u00e8tre de mani\u00e8re \u00e0 ce qu\u2019un ovule f\u00e9cond\u00e9 (le pr\u00e9-embryon) puisse s\u2019y implanter et s\u2019y d\u00e9velopper. En l\u2019absence de conception, le corps lut\u00e9al cesse de fonctionner aux environs du 26e jour du cycle (pour un cycle de 28 jours). Sans le soutien de la progest\u00e9rone, l\u2019endom\u00e8tre commence \u00e0 se d\u00e9composer et est \u00e9limin\u00e9 lors de la menstruation.<\/li>
